Gwinnett County Registration Information
New Student Registration
We are excited your child will be attending a Gwinnett County Public School. Our school system has an easy-to-use, secure online registration system that will allow you to complete many of the forms required for registering your child. Once you complete the online registration portion, you will need to go to your local school to provide additional documentation (stated below) required to enroll your student in a GCPS school.
Important Dates:

Registration Process in 3 Easy Steps
- Complete GCPS Online Registration
- Call the registration office to schedule an appointment for student registration
- Arrive at your scheduled appointment date & time with all of the required paperwork for school registration.
- PLEASE NOTE: Your registration will only be considered complete once we receive ALL the required documents (this also applies to students transferring from a GCPS school).
Items Required for Registration:
- Withdrawal Packet from the previous school, including transfer grades and/or final official transcript
- Proof of residency including 1) a current lease or deed and 2) autility bill
- Student's birth certificate
- Parent ID
- Valid immunization record on Georgia Form 3231 (see specific requirements for 6th Grade and 7th Grade)
- Hearing, Vision and Dental GA form

PLEASE NOTE:
If your child’s registration is incomplete, they will not receive a class schedule until ALL the required documents are on file at Northbrook Middle School. This includes current immunizations on GA Form 3231.
Immunization Updates
By state law, a valid certificate of immunization (Form #3231) or a notarized statement of religious exemption (Form #2208) is required for enrollment. The certificate of immunization must be completed by the health department or your private physician. A valid Form #3231 must be marked with either "Date of Expiration" or as "Complete for School Attendance." (A certificate marked with a "Date of Expiration" expires on the date indicated. A current certificate must be submitted within 30 days of expiration.) A medical exemption, if applicable, should be noted on Form #3231 with a current date of expiration. You may request a form from your school for religious exemption.
Please review immunization requirements for students entering 7th grade and a TDAP-Meningitis parent fact sheet. The new vaccine requirements apply to students who are born on or after 1/1/2002 AND entering 7th grade. The requirements also apply to students who are new entrants to 8th-12th grade. New entrants are students who have never attended any school in Georgia or have not attended any school in Georgia for at least one full school year.
Students also must have evidence of vision, hearing, and dental, and nutrition screening obtained in the past year.This information should be on Georgia Form #3300 (rev. 2013), available from the health department or your physician and dentist and dated within the last 12 months. Letters from appropriate healthcare professionals and out-of-state certificates are acceptable, if completed within the last 12 months and stapled to the state form.
Visit one of the health department offices in Gwinnett County-- located in Buford, Lawrenceville, and Norcross-- to obtain required screenings and immunizations for a fee.
All students entering 6th grade are required to meet the following:
Have two (2) doses of Measles vaccine, two (2) doses of Mumps vaccine, and one (1) dose of Rubella vaccine or laboratory proof of immunity against Measles, Mumps or Rubella.2. Have two (2) doses of Varicella (chicken pox) vaccine or documentation or laboratory proof of immunity.
All students entering 7th grade are required to meet the following:
Have one (1) dose of Tdap (tetanus, diphtheria, pertussis)2. Have one (1) dose of meningococcal conjugate vaccine.Contact your health care provider or local health center to receive any needed vaccinations and to obtain a signed Georgia Certificate of Immunization (Form 3231) with "Complete for School Attendance" marked.

All mandatory forms can be completed online through MyPaymentsPlus . School lunches and school dues can also be paid on MyPaymentsPlus.

Student Withdrawl
We wish your child the best during the transition to a new school. Our school system has an easy-to-use, secure online withdrawal process that will allow you to complete the form required for withdrawing your child. Once you complete and submit the online withdrawal form, you will be contacted by a school representative to verify your ID and to provide you with any additional information necessary to complete the withdrawal if needed for your child.
